Return Merchandise Authorization
Please use the web form below to submit your repair or service requests to UCP’s RMA department. You will receive an email response with an RMA number within 72 hours. Please include a copy of that email with your shipment and clearly write the RMA number on the outside of your shipment. Repair shipments without an RMA number may be rejected, and will be subject to an unauthorized return fee. Customers returning more than 5 devices in a single shipment can download the extended RMA form using the link below. Submit completed forms to email@example.com
